Editable PDF form for user account creation, modification, termination.
Instructions:
- Please open the attached PDF (above), fill out applicable fields, and temporarily Save to your local computer.
- Attach the form and submit to Modern Data. This can be accomplished in two ways. (1) Open a new help desk support ticket via e-mail (support@moderndata.com) or (2) Via the online help desk portal (https://support.moderndata.com).
- Once attached, please delete the file from your computer.
Request definitions:
New Position - new employee (user) or new employee (user) and position.
Change - add, modify, and/or remove permissions/access levels, software, department, location, etc. for an employee (user).
Replacement - new or existing employee (user) replacing an existing employee (user).
Termination - user account removal due to employee (user) separation.
